Abstract

Objective: To evaluate the role of evacuation of hypopyon in the diagnosis and treatment of keratitis. Subject and method: Prospective interventional study without control group of 36 eyes with hypopyon keratitis, including 11 eyes for diagnosis etiology (group 1) and 25 eyes (69.4%) for treatment (group 2). Result: Microbiological result of hypopyon and/or exudates showed that fungi was found in 72.7% and 84% respectively; successful outcome were seen in 81.8% of eyes in group 1 and 60% of eyes in group 2. Of 21 eyes which presented fungi in the anterior chamber in group 2, the ulceration’s size and the hypopyon’s height of eyes with successful outcome was smaller than those of eyes with failure outcome. In group 2, the percentage of successful outcome of eyes with corneal ulcer ≤ 3mm and > 3mm in diameter was 100% and 47.4%, respectively (p=0.028). Conclusion: Evacuation of hypopyon helps to diagnose the cause and to treat keratitis. This surgery should be done in patients who had small ulcer, unchanged exudates with decreased or unchanged hypopyon in anterior chamber after medical treatment.
