The relationship between the value of tumor-infiltrating lymphocytes and pathological complete response in patients with triple-negative breast cancer treated with neoadjuvant therapy

Abstract

Objective: This study aims to evaluate the prognostic value of tumor infiltrating lymphocytes (TILs) for predicting ability of pathologic complete response (pCR). Subject and method: 105 patients with triple-negative breast cancer (TNBC) treated with neoadjuvant therapy at K Hospital. Result: TILs were strongly associated with achieving pCR (OR = 2.41; 95% CI: 1.73 - 3.66). According to receiver operating characteristic (ROC) curve analysis, the AUC was 0.880 for the pembrolizumab group and 0.879 for the chemotherapy group, indicating very good predictive ability of TILs for complete response. With a TILs cut-off of 30%, sensitivity reached 100% in the chemotherapy group, while in the pembrolizumab group, 100% sensitivity was achieved at a TILs cut-off of 50%. Conclusion: These findings suggest that TILs can serve as a predictive biomarker for treatment response in TNBC patients receiving neoadjuvant therapy, in both conventional chemotherapy and immunotherapy combination settings.
