Tóm tắt

Mục tiêu: Nghiên cứu nhằm triển khai mô hình tư vấn sử dụng thuốc từ xa của dược sĩ lâm sàng cho bệnh nhân ngoại trú mắc đái tháo đường típ 2 tại Bệnh viện Hữu Nghị. Đối tượng và phương pháp: Nghiên cứu tiến cứu được thực hiện trên 99 bệnh nhân ngoại trú mắc đái tháo đường típ 2 chưa đạt mục tiêu kiểm soát đường huyết, trong khoảng thời gian từ tháng 4 đến tháng 9 năm 2021. Hoạt động tư vấn từ xa được thực hiện theo phương pháp tư vấn dựa trên vấn đề, tập trung vào tuân thủ điều trị và các vấn đề liên quan đến thuốc (drug-related problem - DRP). Kết quả: Sau can thiệp, tỷ lệ bệnh nhân tuân thủ điều trị (theo MARS-5) tăng từ 51,8% lên 89,3% (kiểm định McNemar, p<0,001), tỷ lệ bệnh nhân gặp ít nhất một DRP giảm từ 67,2% xuống còn 39,7% (p<0,001), chỉ số HbA1c giảm có ý nghĩa thống kê từ 8,8% (IQR: 8,4 - 9,5%) xuống còn 8,3% (IQR: 7,7- 9,3%) (p=0,0054). Tất cả bệnh nhân tham gia khảo sát đều hài lòng với hoạt động tư vấn sử dụng thuốc từ xa. Kết luận: Mô hình tư vấn sử dụng thuốc từ xa cho thấy hiệu quả tích cực trong cải thiện tuân thủ điều trị và kết quả điều trị ở bệnh nhân đái tháo đường típ 2 chưa kiểm soát được đường huyết.
