Pilot study on Remote Medication Counseling by Clinical Pharmacists for Outpatients with Type 2 Diabetes at Friendship Hospital

Abstract
Objective: This study aimed to implement a remote medication counseling model delivered by clinical pharmacists for outpatients with type 2 diabetes mellitus (T2DM) at Friendship Hospital. Subject and method: A prospective study was conducted on 99 outpatients with poorly controlled T2DM at Huu Nghi Hospital between April and September 2021. Remote counseling was provided by clinical pharmacists using a problem-based approach, focusing on medication adherence and drug-related problems (DRPs). Result: Following the intervention, the proportion of patient’s adherent to treatment (assessed using the MARS-5 questionnaire) increased significantly from 51.8% to 89.3% (McNemar’s test, p<0.001). The percentage of patients experiencing at least one DRP decreased from 67.2% to 39.7% (p<0.001). After intervention, the median HbA1c level significantly declined from 8.8% (IQR: 8.4 - 9.5%) to 8.3% (IQR: 7.7 - 9.3%) (p=0.0054). All surveyed patients expressed satisfaction with the remote counseling service. Conclusion: The remote medication counseling model demonstrated positive effects in improving medication adherence and clinical outcomes among patients with poorly controlled T2DM.
