Evaluation of analgesic efficiency of ultrasound-guided femoral nerve block for emergency patients with femoral fracture injuries

Abstract

Summary


Objective: To evaluate analgesic efficiency and side effects of ultrasound-guided femoral nerve block for emergency patients with femoral fracture injuries. Subject and method: A prospective study on 80 emergency patients with femoral fracture injuries at Saint Paul Hospital, Viet Duc Hospital and 103 Military Hospital from March to September 2019 randomized into two groups: Group 1 used ultrasound-guided femoral nerve block with a dose of bupivacain 0.25% 1mg/kg. Group 2 used morphine, intramuscular injection with a dose of 0.2mg/kg, maximum 10mg. All patients were monitored and evaluated in term of analgesic efficiency, impacts on circulation, respiration and other side effects. Result: During 8 hours after analgesia, despite the good analgesic efficiency the VAS at rest in group 2 was higher significantly than that in group 1 with p<0.05 at P5 to H8 time points. Onset time for analgesia of group 1 was shorter than that of group 2 (7.5 ± 3.4 minutes vs 19.8 ± 2.7 minutes, p<0.05). Duration of analgesia of group 1 was longer than that of group 2 (6.28 ± 1.6 vs 4.12 ± 0.59 hours, p<0.05). Patients in both groups did not any changes in circulation and respiration. There was 20% of nausea and vomiting and 10% of urinary retention in group using morphine. Conclusion: Ultrasound-guided femoral nerve block is a safe and good analgesic approach for emergency patients with femoral fracture injuries.
