Abstract

Summary


Objective: To evaluate the effect of empyema treatment by video-assisted thoracoscopic surgery (VATS). Subject and method: The study was conducted on 45 fibrinopurulent and chronic empyema patients who were treated by VATS at Department of Thoracic Surgery of 103 Military Hospital, from January 2017 to April 2019. Methods: Descriptive and prospective study. Result: The mean of age was 54.4 ± 10.5 years, performance status score was 68.6 ± 6.9, concomitant disease include: Diabetes (35.6%), end-stage renal failure (2.2%), lung cancer (2.2%), cirrhosis (2.2%). Results of pleural pus culture with 12/45 (26.7%) was identified pathogenic bacteria. VATS indicated for fibrinopurulent and chronic empyema with the rate of 75.6% and 24.4%, respectively. Treatment methods include: Decortication (55.5%), empyemectomy, debridement, closed drainage (42.4%) and opened drainage (2.2%). The rate of postoperative complication was low (11.1%), wound infections accounted for the majority (44.4%). The rate of good results at the 1st and 3rd month after surgery were 42.2% and 80.0%, respectively. Conclusion: Treatment of fibrinopurulent and chronic empyema by VATS was feasibility, radical treatment by decorticated for subcute stage had hight rate of success. It had the low-rate of complication and the high-rate of good results at the 3rd month after surgery.
