Abstract

Objective: To describe antenatal corticosteroid (ACS) utilization by gestational-age subgroup and to compare neonatal outcomes between women who did and did not receive ACS among preeclamptic women admitted before 34 weeks of gestation. Subject and method: A retrospective cohort study of 247 women with preeclampsia admitted to Hanoi Obstetrics and Gynecology Hospital. Multivariable logistic regression was used to adjust for gestational age at delivery. Result: Overall ACS utilization was 87.4%; among untreated cases, most (54.8%) were in the 32 - 33+6-week group. The no-ACS group had higher admission systolic blood pressure (median 160 vs. 150mmHg) and a shorter admission-to-delivery interval (1.0 vs. 6.0 days, p<0.001). Rates of 5-minute Apgar < 7 (41.9% vs. 16.2%) and neonatal mortality (12.9% vs. 1.4%) were higher in the no-ACS group. After adjustment, non-use of ACS remained independently associated with neonatal mortality (aOR 9.90, 95% CI 1.58 - 62.13) and 5-minute Apgar < 7 (aOR 5.89, 95% CI 1.69 - 20.48). Conclusion: Non-use of ACS was independently associated with adverse neonatal outcomes, particularly below 28 weeks of gestation.
