Abstract

Objective: To evaluate the value of 3.0 Tesla MRI sequences in the diagnosis and classification of post-ganglionic brachial plexus injuries caused by trauma. Subject and method: A cross-sectional descriptive study was conducted on 50 patients with a history of trauma. All patients underwent 3.0T MRI using a protocol of four sequences: T1W Coronal, T2W Sagittal, 3D Axial T2 HSCUBE, and 3D STIR Coronal. Clinical characteristics and the effectiveness of the sequences were analyzed in relation to medical and surgical treatments. Data were processed using SPSS 20.0. Result: Among the 50 patients, the majority were male (male/female ratio: 9/1), with most aged between 20 - 40 years (46%). Traffic accidents were the primary cause (78%). Shoulder abduction and elbow flexion signs showed the highest incidence (20.3%). The 3D STIR Coronal sequence achieved the highest diagnostic performance: in the electromyography (EMG) (+) group, sensitivity was 95.2%, specificity was 73%, and accuracy was 87%; in the EMG (-) group, these values were 92.9%, 37%, and 65.5%, respectively. Conclusion: 3.0T MRI plays a crucial role in accurately assessing the location, morphology, and severity of post-ganglionic brachial plexus injuries. Specifically, the 3D STIR Coronal sequence, when closely integrated with EMG and clinical findings, facilitates effective classification and treatment orientation.
