Long-term functional outcome and prognostic factors after neuronavigation-assisted neuroendoscopic evacuation for supratentorial spontaneous intracerebral hemorrhage

Abstract

Objective: To evaluate long-term functional outcome and identify prognostic factors associated with poor functional outcome in patients with supratentorial spontaneous intracerebral hemorrhage undergoing neuronavigation-assisted neuroendoscopic evacuation at Military Hospital 103. Subject and method: A prospective, descriptive, uncontrolled study was conducted in 51 patients. Functional outcome was evaluated according to the modified Rankin Scale (mRS) at post-operative 6- and 12-months. Logistic regression analysis was performed to determine prognostic factors associated with functional outcome. Result: Poor functional outcome (mRS 4-6) were observed in 43.2% of patients at 6 months and 47.1% at 12 months postoperative follow-up. Factors associated with poor functional outcome included age ≥ 60 years, NIHSS score ≥16, ESSEN score ≥ 6, and the third day postoperative fever. Multivariable logistic regression identified ESSEN score ≥ 6 (OR = 0.148, p=0.006) and hematoma evacuation rate (OR = 0.934, p=0.048) as independent prognostic factors. Conclusion: Long-term functional outcome was primarily determined by admission neurological characteristics and the hematoma evacuation rate.
