Abstract
Objective: To evaluate changes in anterior segment morphological parameters on gonioscopy and AS-OCT before and after Nd:YAG laser peripheral iridotomy (LPI), and to identify morphological predictors of successful outcome. Subject and method: Cross-sectional analytical study of 70 eyes from 35 primary angle closure suspect (PACS) patients at HCMC Eye Hospital (June 2024–December 2025). Gonioscopy and AS-OCT were performed before and 1 month after LPI. Ten AS-OCT parameters (AOD500, AOD750, TISA500, TISA750, IT500, IT750, IC, IC ratio, LV, ACD) were recorded and analyzed using binary logistic regression. Result: Mean age was 57.24 ± 8.1 years; 91.43% were female. One month post-LPI, all angle-opening parameters (AOD500, AOD750, TISA500, TISA750, ACD) increased significantly (p<0.001), while iris morphology parameters (IT500, IT750, IC, IC ratio) and LV decreased significantly (p<0.001–0.04). The regression model identified AOD500, AOD750, TISA750, LV, and ACD as independent predictors of LPI success, with an overall accuracy of 88.6% and AUC = 0.918. Conclusion: Five pre-operative AS-OCT parameters (AOD500, AOD750, TISA750, LV, ACD) demonstrated high predictive value for LPI success, supporting clinical decision-making in the management of PACS patients.
