Comparison of clinical and histopathological features between tumor budding-absent (Bd0) and Low tumor budding (Bd1-4) groups in malignant colorectal polyps at pt1 stage

Abstract
Objective: To evaluate the association between clinicopathological features in pT1 malignant colorectal polyps without tumor budding (Bd0) and those with low-grade tumor budding (Bd1-4). Subject and method: A descriptive cross-sectional study was conducted on 90 cases of pT1 malignant colorectal polyps with tumor budding graded as low or absent, assessed on H&E slides according to the 2016 International Tumor Budding Consensus Conference (ITBCC) guidelines. Clinical, paraclinical, and histopathological features were compared between Bd0 and Bd1-4 groups. Result: Bd0 accounted for 57.8% of cases. There were no statistically significant differences in clinical or paraclinical features between the two groups. However, Bd0 cases showed a higher tendency for endoscopic treatment (63.93% vs. 36.07%, p=0.086). Histologically, the Bd0 group had a significantly higher proportion of well-differentiated carcinomas (grade 1) compared to the Bd1-4 group (84,35% vs 17,65%, p=0,026). Conclusion: Bd0 is associated with more favorable histological features and may serve as a favorable prognostic indicator in pT1 malignant colorectal polyps. Further large-scale studies with long-term follow-up are needed to clarify the clinical value of Bd0.
