Clinical and paraclinical characteristics of delayed drug hypersensitivity at the National Hospital of Dermatology and Venereology

Abstract
Objective: To describe the clinical and paraclinical characteristics of various forms of delayed drug hypersensitivity reactions among inpatients treated at the National Hospital of Dermatology and Venereology. Subject and method: A retrospective cross-sectional descriptive study was conducted from August 2024 to May 2025. All medical records diagnosed with delayed drug hypersensitivity reactions at the National Hospital of Dermatology and Venereology between January 2019 and December 2023 were included in the analysis. Result: A total of 266 medical records were reviewed. The most common clinical manifestations of delayed drug hypersensitivity were erythematous eruptions (95.9%), mucocutaneous erosions (57.1%), and vesiculobullous lesions (45.9%). The presence of bullous lesions was significantly associated with elevated C-reactive protein (CRP) levels (p=0.031) and prolonged hospitalization (p<0,001). Leukocyte abnormalities were detected in 55.3% of cases. Liver and renal function indices showed minor changes, with elevated AST in 22.7%, ALT in 34.2%, urea in 20.7%, and creatinine in 10.9% of patients. Conclusion: Delayed drug hypersensitivity reactions exhibit diverse clinical morphologies. Bullous forms are associated with more significant clinical and paraclinical alterations, underscoring the importance of close monitoring and active management to achieve optimal therapeutic outcomes.
