Evaluating of treatment outcomes in patients with middle cerebral artery occlusion-induced ischemic stroke indicated for decompressive craniectomy at 108 Military Central Hospital

Abstract
To evaluate the treatment outcomes of patients with ischemic stroke due to middle cerebral artery (MCA) occlusion who were indicated for decompressive craniectomy at the 108 Military Central Hospital. Subject and method: A retrospective descriptive study was conducted from January 2022 to December 2024 on 53 patients with ischemic stroke due to MCA occlusion who had indications for decompressive craniectomy at the 108 Military Central Hospital. Treatment outcomes were assessed. Result: At discharge, the mortality rate in the surgical group was 16.1%, compared to 27.3% in the non-surgical group. At 6 months post-discharge, in the surgical group, favorable outcomes were observed in 35.5% of patients, unfavorable outcomes in 35.5%, and mortality in 29.0%. In contrast, the mortality rate in the non-surgical group was 89.9%, compared to 29.0% in the surgical group. Conclusion: Based on observational research, it was found that: At 6 months, favorable outcomes were observed in 35.5% of patients with large middle cerebral artery infarction who underwent decompressive craniectomy, with a mortality rate of 29.0%. In contrast, no favorable outcomes (mRS 0-3) were observed in the non-surgical group, which had a mortality rate of 89.9%.
