A combined clinical and perfusion imaging model to predict functional outcomes after thrombectomy in patients with acute ischemic stroke within the 6-24-hour window

Abstract
Objective: To develop a predictive model for 90-day functional recovery in patients with acute ischemic stroke (AIS) undergoing thrombectomy within a 6–24-hour window, by integrating clinical and perfusion imaging data. Subject and method: A prospective cohort study was conducted at 115 People's Hospital, enrolling 200 patients with AIS from October 2022 to May 2024. Clinical characteristics and brain perfusion imaging data (processed by RAPID software) were collected. A predictive model was developed using multivariable logistic regression. Results: The proportion of patients achieving good functional recovery (mRS 0-2 at 90 days) was 46%. The predictive model, named RAPID PREDICT, incorporated 11 variables and demonstrated excellent discriminatory performance (AUC = 0.891) with good model calibration. The model has been implemented as a web-based clinical support tool, available at https://rapidpredict.com. Conclusion: The RAPID PREDICT model is a valuable tool for early prediction of functional outcomes in patients with AIS undergoing thrombectomy within the 6–24-hour window, supporting personalized treatment planning.
