Interdisciplinary management: Autogenous bone grafting from the retromolar mandible combined with orthodontic treatment in a patient with cleft lip and palate: A case report

Abstract
Cleft lip and palate is a common congenital anomaly that significantly affects orofacial function, facial esthetics, and craniofacial development. In comprehensive management, interdisciplinary coordination, particularly timely orthodontic intervention and secondary alveolar bone grafting, plays a pivotal role in restoring alveolar continuity, correcting dental and arch discrepancies, establishing proper occlusion, and improving masticatory function and facial harmony without the need for prosthetic intervention. This report illustrates the effectiveness of an integrated treatment strategy in a 7-year-old patient with a previously repaired cleft lip and palate who presented with maxillary deformity, anterior crossbite, and a persistent oronasal fistula. The treatment plan combines orthodontic therapy and secondary alveolar bone grafting. Notably, instead of extraoral donor sites such as the iliac crest or calvarium, this case utilized an intraoral donor site from the retromolar mandible, aiming to minimize surgical morbidity while maintaining adequate bone regeneration and providing support for subsequent tooth movement into the cleft area. Following treatment, the patient achieved a Class I occlusal relationship and improved facial balance, facilitating natural tooth eruption and potentially reducing the need for future dental implant placement.
