Study on the results of management of severe kidney injury (grades IV and V) at cho Ray Hospital

Abstract
Objective: Evaluation of the outcomes of managing severe renal trauma (Grade IV and V) at Cho Ray Hospital. Subject and method: This was a retrospective descriptive study on 221 patients with severe renal trauma (Grade IV-V) who were managed at Cho Ray Hospital from January 2019 to December 2022. Variables collected included clinical characteristics, CT scan results, management methods, and rates of early complications. Data was analyzed using Stata 14 software.
Result: Most patients were male (83.7%), with a mean age of 34.66 ± 13.93 years. Traffic accidents were the main cause (78.3%). Grade IV renal trauma accounted for 90.95% of cases, while Grade V accounted for 9.05%. The success rate of non-operative management was significantly higher in Grade IV compared to Grade V injuries (64.18% vs. 40%). Conversely, the rate of emergency surgery was greater in Grade V than in Grade IV patients (35% vs. 2.99%), with most cases resulting in nephrectomy. The overall rate of early complications was 24.88%. Conclusion: Non-operative management is highly effective in hemodynamically stable patients with severe renal trauma. Minimally invasive interventions play a crucial role in kidney preservation. Emergency surgery remains the primary option for life-threatening hemorrhage.
