Long-term survival in patients with early-stage non-small cell lung cancer treated by stereotactic body radiation therapy

Abstract
Background: Stereotactic body radiotherapy (SBRT) is indicated for the treatment of patients with early-stage non-small cell lung cancer (NSCLC) who are not candidates for surgery and has been initially applied in Vietnam. The objective of this study is to evaluate the long-term survival rate after SBRT in patients with stage I NSCLC (T1-T2aN0M0) who are not candidates for surgery. Subejct and method: Prospective study, longitudinal follow-up of 32 patients with stage T1-T2aN0M0 NSCLC, classified according to AJCC version 7.2010, peripheral lung tumors, who received SBRT from January 2015 to November 2021 and then they were followed and evaluated until November 2024. The main criteria were overall survival (OS), survival rates of 3 years, 4 years, and 5 years. Secondary endpoints were update of progression-free survival (PFS) and adverse events. Result: Median PFS was 26 months, 2-year PFS was 53.1%, 3-year PFS was 43.8%, and 4-year PFS was 36.3%. Median OS was 68 months, estimated OS at 3 years was 81.3%, 4-year OS was 64.4%, and 5-year OS was 51%. Patients with a SUVmax value pre-SBRT > 5 had a progression rate of 76.9%, which was statistically significantly higher than patients with SUVmax ≤ 5 at 16.7%, p= 0.005. No additional patients experienced late adverse events. Conclusion: SBRT is a radical treatment that provides good long-term survival and is safe for patients with early-stage NSCLC with peripheral lung tumors inoperable.
