Abstract

Objective: To describe the characteristics of these injuries and evaluate the outcomes of reconstruction of soft tissue defects in the distal one-third of the leg, ankle, and heel using the reverse sural artery flap. Subject and method: A prospective study was conducted on 38 patients treated between January 2011 and July 2013. Patient ages ranged from 18 to 82 years (mean: 50.3 years). Among them, 26 patients (68.4%) were over 60 years old. The cohort consisted of 30 males (78.9%) and 8 females (21.1%). The main causes of injury were traffic accidents (15 cases, 39.5%), occupational trauma (9 cases, 23.7%), postoperative complications (6 cases, 15.8%), and other causes (8 cases, 21.0%). Associated injuries were present in 17 patients (44.8%). Result: Of the 38 cases, 35 flaps (92.1%) survived completely, 3 flaps (7.9%) developed partial necrosis but were salvaged after secondary intervention. No cases of total flap necrosis occurred. Follow-up of 30 patients revealed satisfactory flap coverage, stable wound healing, and aesthetically acceptable scars without major contracture or ulcer recurrence. Functional outcomes were good, with preservation of mobility and weight-bearing capacity. No patients developed donor-site complications, and complete flap survival was achieved in all cases. Conclusion: The reverse sural artery flap is a reliable and effective reconstructive option for soft tissue defects of the distal one-third of the leg, ankle, and heel, providing a high success rate and low complication rate
