Current prophylaxis practices for chemotherapy-induced febrile neutropenia in breast cancer patients at the National Cancer Hospital

Abstract

Objective: To analyze the status and effectiveness of febrile neutropenia (FN) prophylaxis using granulocyte colony-stimulating factor (G-CSF) in breast cancer patients undergoing chemotherapy. Subject and method: A prospective cohort study was conducted on 219 breast cancer patients aged ≥ 18 years receiving chemotherapy at Departments of Internal Medicine 5 and 6, K Hospital, from January 1, 2024, to March 31, 2024. Result:  The mean age was 50.4 ± 10.2 years. The proportion of patients receiving chemotherapy regimens with a high and intermediate risk of FN was 50.7% and 45.7%, respectively. Assessment of prophylaxis according to guideline recommendations showed: appropriate prophylaxis (52.0%), no prophylaxis appropriately indicated (29.2%), over-prophylaxis (15.1%), and under-prophylaxis (3.7%). Most patients received filgrastim (69.7%), with a duration of less than 5 days (95.1%). Thirty patients experienced neutropenia of any grade (13.7%), including 3 patients with severe neutropenia (1.4%). No patients developed FN. Multivariate analysis showed that among patients requiring prophylaxis, appropriate prophylaxis significantly reduced the risk of neutropenia (OR [95% CI]: 0.004 [0.020-0.771]; p=0.033) compared with under-prophylaxis. Among patients not requiring prophylaxis, over-prophylaxis was not significantly associated with neutropenia risk (p=0.418). Conclusion: Adequate G-CSF prophylaxis in accordance with guideline recommendations may provide benefits in reducing the incidence of neutropenia events.
