Abstract

Objective: To evaluate the effectiveness of combining long-pulse laser and intra-scar corticosteroid injection in the treatment of keloid scars and to assess patient satisfaction at 108 Central Military Hospital. Subject and method: A cross-sectional descriptive study with clinical intervention was conducted on 39 patients with keloid scars using convenience sampling. The combined treatment included pulsed dye laser (PDL) or Nd:YAG laser (1064 nm) and intra-scar corticosteroid injection, administered at 4-week intervals. The POSAS (Patient and Observer Scar Assessment Scale), the Goodman and Baron scar grading scale, and a 3-point Likert scale for patient satisfaction were used for evaluation. Result: POSAS scores decreased significantly after 3 months and continued to decrease significantly at 6 months compared to the 3-month mark (p<0.001). After 3 months of treatment, the proportion of grade 4 scars decreased from 64.1% to 28.2%, while grade 3 scars increased from 35.9% to 71.8%. After 6 months, the distribution was grade 1 (33.3%), grade 2 (61.5%), and grade 3 (5.1%), with no grade 4 scars remaining. The overall patient satisfaction rate was 94.9%. Conclusion: The combination of long-pulse laser therapy and intra-scar corticosteroid injections proved effective in the treatment of keloid scars, showing marked improvement after 3 months and further enhancement at 6 months. Patient satisfaction was notably high (94.9%).
