Treatment outcomes and related factors in patients with stage IV non-small cell lung cancer harboring EGFR mutations treated with first-line gefitinib at Vinh Phuc General Hospital

Abstract
Objective: To evaluate progression-free survival (PFS) outcomes and associated related factors in patients with stage IV non-small cell lung cancer (NSCLC) harboring EGFR mutations who received first-line gefitinib. Subject and method: A retrospective–prospective cross-sectional descriptive study with longitudinal follow-up was conducted on 58 patients with stage IV EGFR-mutatant NSCLC who received first-line gefitinib at Vinh Phuc General Hospital between August 2018 and September 2023. Result: The most common age group was 50-70 years, accounting for 70.78%, with an approximately equal gender distribution. Among comorbidities, hypertension was the most common, affecting 18.9% of patients. The highest percentage of metastasis occurred in the pleura and pericardium, accounting for 67.2%. The most common EGFR mutation site was exon 19, which accounted for 63.8%, higher than exon 21. The average CEA concentration after 3 months of treatment decreased from 158.7ng/ml to 43.79ng/ml. The median progression-free survival (PFS) was 9.7 months. In patients with brain metastases, the median PFS was 8.8 months, compared with 10.6 months in those with extracranial metastases. In the group receiving combined radiotherapy, the median PFS (mPFS) was 11.4 months. Conclusion: Gefitinib represents an effective and appropriate first-line treatment option for patients with stage IV NSCLC harboring EGFR mutations.
