Clinical and paraclinical characteristics and 90-day mortality predictors in cancer patients with acute pulmonary embolism

Abstract

Objective: To describe the clinical and paraclinical characteristics and identify independent predictors of 90-day mortality in cancer patients with acute pulmonary embolism (APE). Subject and method: A retrospective descriptive study was conducted on 71 cancer patients diagnosed with APE at the University Medical Center Ho Chi Minh City from February 2020 to April 2023. Multivariable logistic regression analysis was used to determine associations with 90-day mortality. Result: The mean age was 65.0 ± 12.6 years; 56.3% were female. Dyspnea was the most common symptom (66.2%), while 32.4% of cases were incidentally diagnosed. The 90-day mortality rate was 46.4%. The mortality group had a higher heart rate (p=0.041) and lower systolic blood pressure (p=0.092). Positive troponin and elevated NT-proBNP were observed in 53.6% and 60.7% of patients tested, respectively. Lung cancer was the most frequent malignancy (25.3%). A PESI score > 106 was independently associated with mortality (OR = 3.38; 95% CI: 1.05-10.83; p=0.04). Conclusion: A PESI score > 106 is an independent predictor of 90-day mortality in cancer patients with APE, highlighting the utility of this scoring system in early risk stratification and clinical decision-making.
