Graft-versus-host disease following liver transplantation: A case report and therapeutic challenges

Abstract

Graft-versus-Host Disease (GVHD) is a rare but highly fatal complication in liver transplant recipients due to poor response to current treatment strategies. We report a case of a 45-year-old male patient who underwent liver transplantation from a brain-dead donor with an identical ABO blood group at the 108 Military Central Hospital. In the fourth week post-transplant, the patient developed prolonged high fever, generalized skin rash, and diarrhea. Skin biopsy confirmed the diagnosis of GVHD. Despite treatment with interleukin-2 inhibitors and high-dose corticosteroids, the patient showed no clinical improvement. The condition rapidly progressed to septic shock, multi-organ failure, and death. This case highlights the challenges in early diagnosis and effective management of GVHD following liver transplantation.
