Living donor liver transplant for patients with colorectal liver metastases: A case report and literature review

Abstract
Background: Liver is the most common site of metastasis in colorectal cancer, with an incidence of 40-50%, but only 10-20% are eligible for liver resection. Liver transplantation is considered as an alternative in non-resectable colorectal liver metastases. Case report: A 64-year-old, male patient diagnosed with sigmoid colon adenocarcinoma, metastasized to the liver, treated with sigmoidectomy, chemotherapy with 4 cycles of m-FOLFOX + cetuximab regimen, then metastatic liver lesion resection in the 4th segment, followed by 8 cycles of adjuvant chemotherapy. After one year of follow-up, the patient was detected with recurrent liver metastases on bilateral lobes, with elevated CEA, CA 19-9, and was indicated for liver transplant. Post-operative period was uneventful. The patient had frequent follow-up, which was the post-transplant 23rd month, showed free of recurrence and complications. Conclusion: Our case study is the first case of liver transplant for liver metastases in Vietnam, proving the feasibility of liver transplantation as a salvage therapy for the indication. Therefore, to optimize the treatment efficacy on this patient group, more study should be conducted, especially on patient selection criteria.
