Role of 18F-FDG PET/CT for detection of primary tumor in patients with carcinoma of unknown primary

Abstract
Objective: To investigate the detection rate of primary lesions, positive predictive value, location of primary lesions and false-positive images on PET/CT. Subject and method: A retrospective case series study of 67 patients who diagnosed with carcinoma of unknown primary, underwent 18F-FDG PET/CT examination at the Nuclear Medicine Department of Ho Chi Minh City Oncology Hospital from January 2020 to December 2021. Result: A total of 67 patients (54 men and 13 women) with mean age of 56.5 were included in this study. PET/CT suggested the primary tumor site in 46/67 (68.7%) patients with detection rate (DR) 52.2% and positive predictive value (PPV) 0.76. Detection rate of primary tumors in the cervical lymph node metastasis group was higher than the extracervical metastases group (52.7% versus 50%). The most frequent primary tumor found on PET/CT was lung (25.7%), followed by nasopharynx (22.9%). The most frequent false-positive site was tonsils (36.4%). Conclusion: 18F-FDG PET/CT is a technique with good ability to detect unknown primary tumors.
