Abstract

Objective: To evaluate early results of total correction of tetralogy of Fallot in children under 1 year old at Ha Noi Heart Hospital and Thanh Hoa provincial Children's Hospital. Subject and method: A prospective study was conducted on 115 patients at Thanh Hoa Children's Hospital and Hanoi Heart Hospital. Result: The average age of the children was 8.2 ± 2.7 months, 75.7% of the children were 6-12 months old. The mean aortic cross clamp (ACC) time cardiopulmonary bypass (CPB) time were 73.4 ± 22.4 minutes and 99.3 ± 26.9 minutes. The mean aortic cross clamp time and cardiopulmonary bypass time were higher in children under 6 months of age than in the older group (79.8 ± 21.2 and 71.2 ± 22.5 minutes, and 105.9 ± 27.1 minutes and 97.2 ± 26.6 minutes). The length of hospital stay in the group of children under 6 months was lower than the other group (20.1 ± 7.0 and 23.0 ± 17.4 days). The rate of complications and specific complications was higher in the group of children under 6 months compared with the group over 6 months. However, no statistically significant difference was found except for the incidence of pleural effusion and infection. There was no difference in the degree of heart failure, the rate of arrhythmias, the maximum differential pressure across the pulmonary valve as well as the degree of pulmonary regurgitation and tricuspid valve according to age group in the studied pediatric patients at hospital discharge. But the right ventricular dilatation index was higher in the group of 6 months or less than in the larger group. Conclusion: The results of the study show that the total surgical repair of tetralogy of Fallot in children under 1 year of age has a low mortality rate. The results of total surgical repair did not have a significant difference between the group of children 0-6 months and over 6 months-12 months of age.
