Thực trạng tuân thủ hướng dẫn kê đơn dự phòng thuyên tắc huyết khối tĩnh mạch giai đoạn sau xuất viện trên bệnh nhân phẫu thuật chỉnh hình lớn tại Bệnh viện Đa khoa Xanh Pôn

Tóm tắt
Mục tiêu: Đánh giá tuân thủ hướng dẫn kê đơn dự phòng thuyên tắc huyết khối tĩnh mạch trên bệnh nhân phẫu thuật chỉnh hình (PTCH) lớn giai đoạn sau xuất viện. Đối tượng và phương pháp: Nghiên cứu hồi cứu, mô tả trên toàn bộ bệnh nhân thực hiện PTCH lớn tại các Khoa Chấn thương Chỉnh hình, Bệnh viện Đa khoa Xanh Pôn từ 08/4/2026 đến 08/6/2026. Kết quả: Trong 94 bệnh nhân PTCH lớn được đưa vào nghiên cứu, 96,8% (91/94) được kê đơn thuốc chống đông khi ra viện; rivaroxaban 10 mg là thuốc chống đông duy nhất được sử dụng. Tỷ lệ tuân thủ về lựa chọn thuốc là 62,6%; nguyên nhân không tuân thủ do lựa chọn DOAC chưa phù hợp ở nhóm gãy xương đùi. Trong nhóm tuân thủ về lựa chọn thuốc, 100% tuân thủ về liều dùng. Thời gian dự phòng tổng cộng là 29,6 ± 5,6 (ngày) và 100% đạt thời gian dự phòng tối thiểu theo khuyến cáo. Tuy nhiên, chỉ có 66,7% bệnh nhân đạt thời gian dự phòng tối ưu theo khuyến cáo. Kết luận: Thực hành kê đơn dự phòng TTHKTM sau xuất viện đã đạt mức tuân thủ cao về chỉ định, chế độ liều và thời gian dự phòng tối thiểu sau khi hướng dẫn được ban hành; tuy nhiên, vẫn còn tồn tại khoảng trống về lựa chọn thuốc ở nhóm gãy xương đùi và thời gian dự phòng tối ưu, đòi hỏi cần tiếp tục triển khai các biện pháp can thiệp dược lâm sàng.
