Tóm tắt

Mục tiêu: Phân tử điểm kiểm soát miễn dịch CTLA-4 ức chế sự hoạt hóa của tế bào lympho T, đóng vai trò quan trọng trong bệnh sinh, tiến triển của nhiều bệnh truyền nhiễm và ung thư. Nghiên cứu này nhằm đánh giá mối liên quan giữa nồng độ CTLA-4 hòa tan trong huyết tương (sCTLA-4) với các giai đoạn tiến triển bệnh gan ở bệnh nhân nhiễm virus viêm gan B mạn tính. Đối tượng và phương pháp: Nồng độ sCTLA-4 huyết tương được định lượng bằng phương pháp elisa trong nghiên cứu cắt ngang có nhóm chứng gồm: 74 bệnh nhân viêm gan mạn tính, 76 bệnh nhân xơ gan, 72 bệnh nhân ung thư biểu mô tế bào gan và 72 người khỏe mạnh làm nhóm chứng. Kết quả: Nồng độ sCTLA-4 huyết tương ở nhóm bệnh nhân cao hơn có ý nghĩa so với nhóm chứng; tăng dần ở nhóm viêm gan mạn tính, xơ gan và ung thư gan. Nồng độ sCTLA-4 tương quan thuận với chỉ dấu đánh giá mức độ tổn thương gan như AST, ALT, Fib-4. Kết luận: Nồng độ sCTLA-4 tăng ở các giai đoạn tiến triển của bệnh gan liên quan với nhiễm virus viêm gan B. sCTLA-4 có thể là một dấu ấn miễn dịch tuần hoàn bổ trợ trong quản lý bệnh nhân nhiễm HBV mạn tính.
