Abstract

Objective: To review and analyze the technical parameters of diode laser used in the treatment of hemorrhoids to identify the optimal configuration regarding wavelength, power, emission mode, and total energy that maximizes efficacy and minimizes complications. Subject and method: A systematic literature review was conducted using PubMed, Google Scholar, and ResearchGate to identify studies published between January 2023 and December 2024. A total of 29 studies meeting inclusion criteria were analyzed for laser settings, clinical outcomes, and complications. Result: The 1470nm diode laser was most used wavelength, associated with high efficacy and low complication rates. The optimal configuration included a 1470nm wavelength, power setting of 6-10W, 3-second pulse duration, pulsed emission mode, and total energy of 150-250J per hemorrhoidal pile. Grade IV hemorrhoids showed a higher complication rate when treated with high energy or continuous mode. Conclusion: Laser hemorrhoidoplasty is an effective and minimally invasive treatment for grade II–III internal hemorrhoids when appropriate laser parameters are applied. Further standardized trials are needed to develop optimal clinical guidelines.
