Medication review by pharmacist on outpatients with type 2 diabetes at Duc Giang General Hospital

Abstract
Objective: The study aims to describe medication reviews by pharmacists and investigate the effects of the medication reviews on glycemic outcomes in type 2 diabetes (T2DM) outpatients with poor glycemic control. Subject and method: T2DM outpatients with an HbA1c of ≥ 8.5% in October 2023 were included for the medication review. Pharmacists reviewed medications to identify DRPs, discussed with doctors to reach a consensus, and informed doctors of detected DRPs in a professional meeting. The effects of this activity on glycemic outcomes were assessed by comparing the HbA1c before (T0) and after (T2 and T3) the medication review with the HbA1c at medication review (T1). Result: 186 patients were included for the medication review. There were 156 DRPs in total, the median number of DRPs per patient was 1, and DRPs of drug selection were the most prevalent (47.9%). Before medication review, HbA1c at T0 was significantly lower than that at T1 while HbA1c significantly improved with a decrease of 0.36% and 0.47% at T2 and T3 after medication review, respectively. Conclusion: Medication review by pharmacists can help improve glycemic outcomes in T2DM patients with poor glycemic control.
