Recurrence patterns following surgical resection of stage I and II non-small cell lung cancer: A prospective analysis

Abstract

Objective: To identify patterns of recurrences in patients with stage I and II non-small cell lung cancer (NSCLC) following surgical resection. Subject and method: Between June 2017 and May 2023, sixty - three patients with stage I or II NSCLC underwent video-assisted thoracoscopic (VATS) lobectomy and systematic lymph node dissection at 108 Military Central Hospital. Recurrent patterns were documented and analyzed. Cross-tabulation analysis utilizing chi-square and Fisher's exact tests were performed to assess associations between recurrent patterns and clinicopathological variables, including nodal involvement and primary tumor features. Result: During the follow-up period, the overall proportion of recurrence was 23.8%. Among patients with recurrences, multiple sites of recurrence were the most common (53.4%). Higher recurrence rates were observed in patients with ≥2 positive lymph nodes, or involvement of ≥2 nodal stations, or primary tumors >3 cm in tumor diameter. Conclusion: The extent of lymph node involvement (both number of positive nodes and nodal stations) and primary tumor size were significantly associated with recurrence in NSCLC patients at stage I and II postoperatively
