Abstract

Objective: To study the value of the each characteristics of ultrasound imaging in the diagnosis thyroid cancer such as hypoechoic, irregular margins, extra-thyroidal extension, height > width and microcalcifications. Subject and method: This study included 696 patients with 717 thyroid nodules confirmed by histopathology, from September 2019 to August 2020. All patients were evaluated by ultrasound with US machine (GE Logiq S8, US) and cytology with result using Bethesda classification 2017. Result: Of 717 thyroid nodules included 584 malignant (10.6 ± 7.3mm) and 133 benign (22.1 ± 12.9mm). The pathology included 94 nodules of (70.7%), 6 nodules of focal chronic disease (4.5%), 33 nodules of adenoma (24.8%) in the benign and 187 nodules of micro papillary thyroid carcinoma (32.0%), 397 nodules of papillary thyroid carcinoma, 13 nodules of papillary-follicular variant thyroid carcinoma and other variants (2.2%), 5 nodules of follicular thyroid carcinoma (0.9%) in the malignant. The ultrasound imaging of the 584 malignant nodules included 520 nodules with hypoichoic (89%), 265 nodules with irregular margins (45.4%), 121 nodules with extra-thyroidal extension (20.7%), 219 nodules with height > width (54.6%) and 165 nodules with microcalcifications (28.3%). The diagnostic value for thyroid cancer with the sensitivity, and specificity of each characteristic with hypoechoic, irregular margins, extra-thyroidal extension, height > width, microcalcifications were 89% and 63.1%, 45.3% and 84.2%, 30.7% and 97%, 54.6% and 83.5%, 5%, 45.3% and 84.2%. Conclusion: The sensitivity and specificity of the imaging features on ultrasound of the hypoechoic, irregular margins, extra-thyroidal extension, the height than width and microcalcifications were 89% and 63.1%, 45.3% and 84.2%, 20.7% and 97%, 54.6% and 83.5%, 28.3% and 93.2%.
