Main Article Content
Abstract
Clinical pharmacist (CP) counselling activity may have positive impact in improving knowledge of patients about oral anticoagulants and antiplatelets (OAAs). Objective: To assess the knowledge of patients who were prescribed OAAs during hospitalization before and after the CP-provided education. Subject and method: A descriptive prospective study was conducted at Vinmec healthcare system. Result: The median knowledge score of 91 recruited patients was 22.2 (0 - 44.4) (100 score scale) before counselling activity; percentage of patients who had right answer for each individual knowledge question was 0 - 45.1%. After educational interview by CPs, the patients’ knowledge score increases significantly by median 62.5 (33.1 - 88.9) with p<0.001, 12 in 13 individual knowledge items were answered right by more than 80% patients. Conclusion: Involvement of CPs in patient education played an important part to improve the limited knowledge of patients under longterm OAA therapy.
