Study on the hemodynamic impact of atrial fibrillation in patients with pulmonary hypertension due to left heart disease

Abstract
Objective: To determine the hemodynamic impact of atrial fibrillation (AF) in patients with pulmonary hypertension (PH). Subject and method: Prospective and cross-sectional descriptive method carried on 67 patients with confirmed pulmonary hypertension related to left heart disease who were isolated mitral valve replacement or simultaneous aortic and mitral valve replacement. Evaluated hemodynamic parameters by transthoracic echocardiography and Swan-Ganz catheter insertion method. Studied time was from April 2017 to April 2018. Result: Atrial fibrillation was prevalent in 46.3%. Invasive hemodynamic parameters consist of PAPs, PAPm, PAOP, TPG, PVR, RAP, CI and noninvasived parameters included LAd, TAPSE, EF in PH with AF patients were significantly compromised compared to patients with PH and sinus rhythm. Conclusion: PH was associated with increased prevalence of AF. Occurrence of AF in PH indicated clinical deterioration and worse hemodynamics. PAPs, PAPm, PAOP, TPG, PVR, RAP, CI and LAd, TAPSE, EF parameters in PH with AF group were changed significantly compared to PH with sinus rhythm (p<0.05).
