Results of treatment of osteoporotic vertebral compression fractures by vertebroplasty/kyphoplasty

Abstract

Objective: To evaluate results of treatment the patients with osteoporotic vertebral compression fractures (OVCFs) by vertebroplasty (Percutaneous vertebroplasty - PVP & Percutaneous Balloon Kyphoplasty - PKP). Subject and method: Retrospective review of 149 patiens with OVCFs were treated by vertebroplasty (PVP & PKP). The analgesic effects were assessed on a VAS scale at pre-intervention intervals, 24 hours and 3 months after intervention. Changes of the kyphotic wedge angle, increased the postoperative vertebral body height were assessed on X-ray film in comparison between pre- and post-intervention. Results of the intervention were assessed by Macnab standards. Complications during and after intervention were recorded. Result: 149 patiens with OVCFs on one level (include 40 men and 109 women, the average age were 75.13 ± 11.26) were treated by PVP = 29 cases & PKP = 120 cases. The positions of OVCFs were: D12 = 39 cases, L1 = 74 cases, L2 = 20 cases and 16 cases in other. The mean VAS score: at pre-intervention = 8.02 ± 0.69; post-intervention 24 hours = 2.03 ± 0.75; post-intervention 3 months = 1.13 ± 0.34. The result according to Macnab standards: 51.67% were very good; 44.3% were good and 4.03% were average. The kyphotic wedge angle, and the postoperative vertebral body height were improved in PKP group (p<0.001). There were 67 cases (45%) with leakage of cement out of vertebral body, neither spinal-cord nor nerve-root was injuried. Conclusion: Vertebroplasty (PVP & PKP) to treat OVCFs is a safely procedures, fewer complications, that reducing back pain and improving the quality of life. The very good and good result were 95.97%.
