Abstract

The abdominal aortic aneurysm which is located above the renal arteries and below the celiac artery is the most difficult position to manage and treat, therefore it requires perfect management and treatment strategies. We would like to report and share therapeutic tactics for a case of saccular abdominal aortic aneurysm above renal arteries and below celiac artery combined with a superior mesenteric artery occlusion that has undergone cutting aortic aneurysm and reforming aorta successfully. 34 year-old male patient, with no history of hypertension, admitted to hospital for having stomachache around the navel. On multi-slice computed tomography (MSCT) images, a saccular bulge at the abdominal aortic artery was detected that is situated 5mm above the right renal artery; 12mm below the celiac artery; sizes 44.8 × 48.5mm and its neck was 17.6mm wide. We implemented the surgery on abdominal-thoracic line with left diaphragm-ectomy, then used temporary shunt of the upper and lower ends of the bulge sac, cut the aneurysm and finally reformed the aorta with dacron patch. The patient was then stable and recovered quickly, discharged 10 days after surgery.
