Evaluating the early treatment results of video-assisted thoracoscopic surgery for lung diseases with peripheral solitary nodule

Summary
Objective: To evaluate the early treatment results of video-assisted thoracoscopic surgery (VATS) application for lung diseases with peripheral solitary pulmonary nodule. Subject and method: A prospective and description study of 135 patients with peripheral solitary pulmonary nodules whom were treated by VATS at 103 Military Hospital and Pham Ngoc Thach Hospital, from November 2011 to October 2019. Result: The patients’ average age was 52.3 ± 11.4 years, the male/female ratio was 1.3/1. Lung resection type was performed according to the results of frozen-section histological examination, of which 51 patients with benign nodules underwent wedge-resection, lobectomy and and lymph nodes harvasting (average number of N2 lymph nodes was 1.0 ± 0.8, N1 was 1.1 ± 0.3 per one patient) were performed in 84 non-small cell lung cancer patients. Mean surgical time and blood loss were 104.7 ± 52.7 minutes and 110.2 ± 125.4ml, respectively. The rates of serious bleeding and blood transfusion during surgery wewe 3.7% and 4.4%, respectively. Postoperative results: Mean intravenous analgesics medication time was 2.6 ± 0.8 days, low and moderate pain was most seen at the 5th day post surgery (97.9%), average chest tube drainage times and hospital stay were 3.1 ± 1.9 days and 7.1 ± 2.2 days. The rate of postoperative complications was 11.1%, of which majority was prolonged air leakage (5.2%). Conclusion: VATS application for peripheral solitary nodule treatment was safe with good early results.
