Influenced factors on epithelial healing time of corneal persistent epithelial defects treated by inlay amniotic membrane transplantation

Abstract

Summary


 Objective: To evaluate the factors that influenced on the epithelialization time of corneal persistent epithelial defect (PED) eyes treated by amniotic membrane transplantation (AMT). Subject and method: A retrospective study that described the clinical intervention with longitudinal comparison (before and after treatment) on 89 eyes (87 patients) with corneal PEDs from 2009 to 2018 at 103 Military Hospital. Result:  The study included 87 eyes of 85 patients (48 males and 39 females), aged from 17 to 85 years (63.7 ± 20.9). The epithelialization times after inlay AMT were from 4 to 25 days (16.3 ± 10.1). Comparing epithelialization time among different causes, the PEDs caused by infection had the shortest epithelial healing time and the PEDs caused by burns and immune-related factors had the longest epithelial healing time (p<0.05). The duration of corneal ulceration was positively correlated with the duration of epithelial healing (r = 0.57, p<0.05). Ulcer size and depth were almost unrelated to epithelial healing time (r < 0.2, p>0.05). In combination lesions, tear secretion, meibomian gland dysfunction (MGD) and corneal sensation were significantly associated with epithelial healing time (p<0.05). The epithelial healing time was longer in group of limbal stem cell deficiency with 180 degrees or more than the remaining groups (p<0.05). Conclusion: Inlay AMT was an effective treatment for corneal PED. Preoperative factors such as the duration of corneal ulcer, the cause of corneal ulcer, dry eye, MGD and corneal sensation were prognosis factors for surgical outcome.
