Abstract
Objective: This study aims to investigate drug-related problems (DRP), their potential clinical impact, and associated factors in hospital discharge prescriptions among patients with acute coronary syndrome (ACS). Subject and method: A descriptive cross-sectional study was conducted on discharge prescriptions of patients with ACS at a general hospital in Ho Chi Minh City from March 2025 to December 2025. DRP were identified and classified according to PCNE version 9.1, and their potential clinical impact was assessed using the HAMEC tool. Factors associated with the occurrence of DRP were analyzed using multivariable logistic regression. Result: Among 401 prescriptions, 461 DRPs were identified, and 67.1% of prescriptions had at least one DRP. DRP related to medication administration accounted for the largest proportion (58.6%), mainly due to incomplete or inappropriate instructions for medication administration (51.8%), followed by DRP related to drug selection (29.3%), of which inappropriate drug combinations accounted for 11.9%. Most DRPs were classified as having a minor potential clinical impact (73.3%). The number of comorbidities was independently associated with the occurrence of DRP (OR = 1.296; 95% CI: 1.104 - 1.521, p=0.001). Conclusion: DRP were common in discharge prescriptions for patients with ACS, predominantly involving instructions for medication use and drug selection. Strengthening prescription review and the role of clinical pharmacists is warranted to minimize preventable DRPs.
