Abstract

Objective: To describe the characteristics of prophylactic antibiotic use in patients undergoing cesarean section at the Department of Reproductive and Obstetric Health Care, Hai Ha District Medical Center. Subject and method: A retrospective descriptive study was conducted on 242 medical records of patients who underwent cesarean section at the Department of Reproductive and Obstetric Health Care, Hai Ha District Medical Center, from August 1, 2023, to July 31, 2024. Eligible participants were hospitalized patients prescribed antibiotics without any pre-operative diagnosis of infection. Result: A total of 242 eligible patients were included in the study. The median age was 28 years (IQR: 23 - 33), with the primary indication for cesarean section being a history of prior cesarean delivery (54.1%). Prophylactic antibiotics were administered within 1 hour prior to skin incision in 98.3% of patients, with cefazolin being the most prescribed agent (82.2%). Discontinuation of antibiotics within 24 hours post-operatively was recorded in 67.0% of cases, whereas 32.2% of patients received prolonged antibiotic therapy for 5 days or more post-operatively. Factors statistically significantly associated with prolonged antibiotic duration included history and number of previous cesarean sections, emergency surgery, ASA ≥ 3, amniotic fluid abnormalities, maternal comorbidities, concurrent surgical procedures during cesarean delivery, and dense adhesions from previous cesarean scars. Conclusion: The rate of appropriate surgical antibiotic prophylaxis according to ASHP guidelines at the health center was relatively high. However, updating institutional guidelines is warranted to further optimize surgical antibiotic prophylaxis in cesarean deliveries.
