Abstract

Objective: To evaluate the cost-effectiveness of adding dapagliflozin to standard of care (SoC) compared with standard of care alone in patients with heart failure with preserved ejection fraction (HFpEF) in Vietnam. Subject and method: The analysis was conducted from the payer’s perspective, using a Markov model based on four quartiles of the KCCQ-TSS, with a one-month cycle length and a lifetime horizon. Data on effectiveness, safety, and quality of life were obtained from previously published studies. Sensitivity analyses were performed to assess the robustness of the results. Result: Adding dapagliflozin increased costs by VND 34.6 million per patient, mainly due to drug costs, but this was partially offset by reductions in costs related to acute events, mortality, and adverse events. In terms of effectiveness, dapagliflozin provided an additional 0.3147 QALYs. The estimated ICER was VND 109.9 million per QALY, which was lower than the willingness-to-pay threshold in Vietnam (VND 342 million per QALY). Sensitivity analyses showed that the ICER remained stable under variation in input parameters. Conclusion: Adding dapagliflozin to standard of care is a cost-effective treatment strategy for the management of HFpEF in Vietnam.
