Abstract

Objective: To evaluate the effectiveness of clinical pharmacy activities in managing linezolid (LNZ) use following the implementation of the Linezolid Use Guideline at Military Central Hospital 108. Subject and method: A before-and-after interventional study was conducted among hospitalized patients prescribed linezolid. The evaluation included 322 patients retrospectively identified in 2024 and 71 patients in the post-intervention period who had a linezolid request form and received clinical pharmacist interventions. Results and conclusion: Of the 94 patients receiving linezolid, 71 had a linezolid request form, all of which were reviewed and approved by clinical pharmacists prior to linezolid initiation. Clinical pharmacy interventions significantly improved the appropriateness of linezolid prescribing, with the proportion of appropriate indications increasing from 38.51% to 82.69% and the proportion of priority indications according to the Linezolid Use Guideline increasing from 53.11% to 80.77%. The proportion of linezolid use as an alternative to vancomycin/teicoplanin increased from 8.73% to 19.23% (p<0.05). The proportion of prescriptions for indications not included in the Linezolid Use Guideline decreased from 10,56% in the retrospective period to 1.92% in the prospective period. All serious drug-drug interactions were identified and addressed through clinical pharmacist interventions. Notably, interventions recommending modification of concomitant medications achieved a 100% physician acceptance rate. These findings demonstrate that a linezolid stewardship program incorporating clinical pharmacy activities is a feasible approach that contributes to improving the quality and appropriateness of antibiotic use and supports the hospital antimicrobial stewardship program.
