Abstract

Objective: To describe the imaging characteristics of ruptured middle cerebral artery aneurysms on CTA and evaluate the outcomes of microsurgical clipping at Military Hospital 103. Subject and method: A retrospective cross-sectional descriptive study was conducted on cases of ruptured middle cerebral artery aneurysms that underwent microsurgical clipping between October 2023 and October 2024. Result: The majority of cases presented with subarachnoid hemorrhage (95.8%). Intracerebral hemorrhage was noted in 26.1% of cases. Fisher grade II subarachnoid hemorrhage was the most common, accounting for 47.8% of cases. Aneurysms at the M1 - M2 bifurcation were most frequent (87%). Most aneurysms (56.5%) measured 5 - 10mm in diameter, and 52.2% had a neck diameter ≥ 4mm. Complete aneurysm occlusion with clipping was achieved in 91.3% of cases. Conclusion: Microsurgical clipping is a safe and effective treatment with a high success rate for the complete obliteration of ruptured aneurysms, as demonstrated in a study of 23 patients evaluated via cerebral computed tomography angiography (CTA) at 103 Military Hospital.
