Abstract
Background: Intertrochanteric femoral fractures in the elderly are associated with high mortality and complication rates. Gamma 3 nail fixation under fluoroscopic guidance is a minimally invasive technique that has not been systematically studied at provincial hospitals. Objective: To describe the clinical and radiographic characteristics and evaluate the outcomes of surgical treatment of intertrochanteric femoral fractures using the Gamma 3 nail under fluoroscopic guidance at Binh Thuan General Hospital. Subject and method: A non-controlled clinical interventional study was conducted on 50 patients aged ≥60 years with closed traumatic intertrochanteric femoral fractures, surgically treated at Binh Thuan General Hospital from June 2025 to December 2026. Result: The mean age was 78.90 ± 9.12 years; 70.0% were female. The main cause of injury was domestic accidents (94.0%). Type A2 fractures predominated (88.0%); 68.0% had lateral wall involvement. The mean operative time was 62.34 ± 21.20 minutes; the mean blood loss was 202.00 ± 63.86 ml. Closed reduction was successful in 90.0% of cases; postoperatively, 68.0% achieved a neck-shaft angle >130°. Reduction quality was good in 20.0%, acceptable in 78.0%. Conclusion: Minimally invasive fixation with the Gamma 3 nail under fluoroscopic guidance is a feasible and safe technique with short operative time, low blood loss, and a high rate of satisfactory reduction quality in elderly patients.
