Functional outcomes and bone union after pfna surgery for intertrochanteric femoral fractures: A report of 36 cases with final follow-up at Nam Sai Gon International General Hospital

Abstract

Objective: To evaluate functional outcomes and bone union after osteosynthesis with a proximal femoral nail antirotation (PFNA) for the treatment of intertrochanteric femoral fractures at Nam Sai Gon International General Hospital. Subject and method: This was a retrospective descriptive case series of 36 patients with intertrochanteric femoral fractures who underwent PFNA osteosynthesis from September 2022 to September 2023 and had complete final follow-up data. The assessed outcomes included bone union, neck-shaft angle, tip-apex distance (TAD), complications, and hip function according to the Harris Hip Score (HHS). The mean follow-up duration was 26.8 months. Result: The mean age was 68.8 ± 16.6 years, and males accounted for 63.9%. The main mechanism of injury was a fall from standing height, accounting for 97.2%. According to the AO/OTA classification, fracture types were A1 in 25.0%, A2 in 69.4%, and A3 in 5.6% of patients. The mean operative time was 74.3 ± 24.6 minutes, and the median blood loss was 100mL [50; 200]. At the final follow-up, bone union was recorded in 36/36 patients; 100% had a neck-shaft angle within 120 - 130°; and the mean TAD was 22.7 ± 5.6mm. Good and excellent HHS results were achieved in 32/36 patients, accounting for 88.9%. No early or late complications were recorded in the followed-up patients. Conclusion: In this group of 36 patients with intertrochanteric femoral fractures and complete follow-up data, PFNA osteosynthesis showed a high rate of bone union, good restoration of the neck-shaft angle, and favorable hip functional outcomes.
