Abstract

Objective: To describe medication adherence and factors associated with high adherence among senior military officers with hypertension at the 108 Military Central Hospital. Subject and method: A descriptive cross-sectional study with analytical components was conducted among 350 senior military officers with primary hypertension receiving outpatient care at the Department for Senior Military Officers from June 2025 to December 2025. Medication adherence was assessed using the 8-item Morisky Medication Adherence Scale (MMAS-8); high adherence was defined as an MMAS-8 score of 8. Multivariable logistic regression was applied to identify associated factors. Result: The mean age was 68.75 ± 7.6 years; 71.1% were aged ≥ 60 years and 98.0% were male. The proportion achieving the blood pressure target of < 130/80mmHg was 36.6%. Overall adherence (MMAS-8 ≥ 6) was 84.9%, including high adherence in 34.6%, moderate adherence in 50.3% and low adherence in 15.1%. The most common barriers were occasionally forgetting to take medication (54.3%) and forgetting to bring medication when travelling (27.4%). Patients aged 45-59 years were more likely to achieve high adherence than those aged ≥ 60 years (aOR = 2.169; 95% CI: 1.040 - 4.524; p=0.039). Conclusion: Medication adherence was relatively high, but high-level adherence and target blood pressure control remained suboptimal. Individualized counselling, reminder strategies and regimen simplification should be prioritized, especially for older patients with multimorbidity and polypharmacy.
