Abstract

Objective: To describe clinical and paraclinical characteristics and evaluate the outcomes of endovascular thrombectomy in patients aged ≥ 65 years with acute ischemic stroke due to large vessel occlusion. Subject and method: A cross-sectional descriptive study combined with longitudinal follow-up was conducted on 139 patients aged ≥ 65 years diagnosed with acute ischemic stroke due to anterior circulation large vessel occlusion, who underwent endovascular intervention at 108 Military Central Hospital during 2023 - 2024. Result: The mean age was 67.4 ± 10.7 years, with males accounting for 66.9%. The mean NIHSS score at admission was 14.3 ± 6.5; pre-stroke mRS was 0.7 ± 1.2, and ASPECTS was 8.0 ± 1.5. The most common occlusion site was the middle cerebral artery (32.4%). Moderate or poor collateral circulation was observed in 76.3% of patients. The successful recanalization rate (mTICI 2b-3) was 97.1%, and in-hospital mortality was 6.5%. Conclusion: Patients aged ≥ 65 years often present with moderate to severe stroke and limited collateral circulation. However, endovascular thrombectomy remains an effective treatment, with a high recanalization rate and relatively low in-hospital mortality.
