Diagnostic value of computed tomography with rectal contrast administration in detecting colorectal anastomotic leakage after cancer surgery

Abstract
Objective: To determine the diagnostic value of CT with rectal contrast administration in detecting colorectal anastomotic leakage after surgery for colorectal cancer. Subject and method: A retrospective cross-sectional study was conducted in 80 patients with clinically suspected colorectal anastomotic leakage after cancer surgery, who underwent CT with rectal contrast administration and subsequent reoperation. CT findings were compared with operative findings to evaluate diagnostic performance. Results: The mean age was 58.1 ± 16.2 years, and men accounted for 60.0% of the cohort. The absence of free intraperitoneal air had the highest rule-out value for anastomotic leakage (LR- = 0.10; 95% CI: 0.04 - 0.26). Perianastomotic free air had the highest rule-in value (LR+ = 8.29; 95% CI: 1.29 - 53.36). CT with rectal contrast administration showed a sensitivity of 92.9%, a specificity of 70.0%, a positive predictive value of 95.6%, and a negative predictive value of 58.3% (LR+ = 3.10; LR- = 0.10). Conclusion: CT with rectal contrast administration was a useful modality for diagnosing colorectal anastomotic leakage after cancer surgery, particularly in patients with persistent clinical suspicion. However, a negative CT result did not completely exclude anastomotic leakage and should be interpreted in conjunction with the clinical findings.
