Results of transperitoneal laparoscopic donor nephrectomy: A 500-case series at 108 Military Central Hospital

Abstract

Objective: To evaluate the clinical characteristics, surgical techniques, and outcomes of laparoscopic living donor nephrectomy at 108 Military Central Hospital. Subject and method: A total of 501 living donor nephrectomies performed at 108 Military Central Hospital between 2016 and 2025. Analyzed variables included donor characteristics, renal vascular anatomy, operative time, blood loss, complications, and comparisons with other transplantation centers. Result: The mean donor age was 31.74 ± 7.99 years, with females accounting for 33.4% of cases. The left kidney was predominantly selected (80.5%). Multiple renal arteries (≥ 2 arteries) were identified in 19.3% of donors. The mean operative time was 96 ± 25 minutes, estimated blood loss was 102 ± 40mL, and warm ischemia time was 3.3 ± 0.8 minutes. The rates of Clavien-Dindo grade I-II and grade III-IV complications were 2.5% and 0.8%, respectively. No patients required blood transfusion, and the conversion rate to open surgery was 0.2%. Conclusion: Laparoscopic living donor nephrectomy is a safe and effective procedure associated with low complication rates and can be routinely implemented in major transplant centers.
