Vertebral fractures in women aged over 65 years with type 2 diabetes mellitus and osteoporosis at Nguyen Tri Phuong Hospital

Abstract
Background: Vertebral fracture is a serious complication of osteoporosis, leading to disability and reduced quality of life in the elderly. Type 2 diabetes mellitus has been shown to increase fracture risk through alterations in bone metabolism at the cellular level. In older women, the coexistence of type 2 diabetes mellitus and osteoporosis may further increase the risk of vertebral fractures. However, in Vietnam, data on this issue remain limited. Objective: To determine the prevalence and radiographic characteristics of vertebral fractures on X-ray, as well as the risk factors predicting vertebral fractures in women aged over 65 years with type 2 diabetes mellitus and osteoporosis. Subject and method: A cross-sectional study was conducted on 98 women aged over 65 years with type 2 diabetes mellitus and osteoporosis who were enrolled at the Endocrinology Department and the Rheumatology Department of Nguyen Tri Phuong Hospital from October 2024 to June 2025. Result: The prevalence of vertebral fractures was 14.3%. Most patients had a fracture at a single vertebra, with the T12 vertebral body being the most commonly affected site. The predominant fracture type was wedge fracture (81.25%), and severe fractures accounted for 50%. Two independent risk factors associated with vertebral fractures were duration of diabetes mellitus (OR = 3.9, p=0.008) and diabetic retinopathy (OR = 7.3, p=0.006). Conclusion: Elderly women with type 2 diabetes have a significantly increased prevalence of vertebral fractures, particularly among those with diabetic retinopathy and a longer duration of diabetes.
